Interdisciplinary Assesment

Our 360 degree assessment is completed by our core teams of a Physician, Psychologist, Occupational Therapist and Physical Therapist. This integrated biopsychosocial assessment, will allow our team to understand you, your condition and how it is impacting all aspect of your life.

Completed over two days, the team will identify your barriers to recovery and develop a treatment plan to help you achieve your goals.

Our team has completed thousands of assessments and look forward to meeting you to determine how we can help. Please know: some people come to our clinic with optimism, others with a sense of dread, fearful of another medical intervention, others just don’t know what to expect. Whatever you are feeling as you contemplate an assessment with us, our clinicians are prepared and look forward to hearing your concerns and assisting you in your recovery. What to expect Our assessments are typically completed over two days, please dress in loose comfortable clothes and wear closed-toe/heel shoes that are flat and supportive. A water bottle is recommended, and we have kitchen/washroom facilities for your use. Your assessment will be designed specifically to your needs and current diagnosis. You will meet with our Physician, Psychologist, Occupational Therapist and Physical Therapist. Sessions with each clinician are typically one to two hours in duration, and they will strive to limit unnecessary symptom aggravation. The team will explore every aspect of your condition and how it is impacting your daily life and routine. Your goals and expectations will be discussed and used to create a treatment plan to help you maximize your potential. If the team feels our services are not right for you—we will also make other recommendations accordingly. Following the assessment, if you have questions, we are happy to review our clinical findings and recommendations with you to help you understand the proposed treatment plan.

Clients that attend our programming/services have often had multiple treatment providers and multiple interventions for their condition and are often seeking medical or hands-on treatments. Our goal is to have a holistic, biopsychosocial approach to your client’s care to ensure we deliver integrated, evidence-based and informed treatment under one roof. Where indicated, additional resources can be added to our assessment team (Pharmacy or Psychiatry) to better inform treatment recommendations and meet your client’s needs.

Our teams are experienced in trauma informed care and make efforts to approach assessment and treatments with some sensitivity, including attention to the environment, touch, tone, vocabulary, pace and repetition of information.

The flow and emphasis of the assessment is dependent on your client’s primary diagnosis(es), and as such no initial assessment will be quite the same as the next. Our clinicians have years of experience working in the field of mental health disorders, addiction and chronic pain and are passionate about the mind-body connection and its role in recovery.

Our clinical care coordinator will discuss your expectations prior to assessment and then communicate with you following the initial assessment regarding clinical findings, the identified barriers, treatment recommendations and anticipated outcome of any proposed services. A detailed intake assessment report will be completed by the team and issued within 10 business days of the assessment.
